To: Exam applicants for the 08 March 2026 Career Service Examination – Pen and Paper Test (CSE‑PPT) in Calapan City, Oriental Mindoro who successfully booked an appointment 𝘃𝗶𝗮 𝗢𝗖𝗦𝗘𝗔𝗦
1. Bring with you the following:
– Printed application form (𝟴.𝟱𝗶𝗻 𝘅 𝟭𝟯𝗶𝗻) 𝗴𝗲𝗻𝗲𝗿𝗮𝘁𝗲𝗱 𝗶𝗻 𝘁𝗵𝗲 𝗢𝗖𝗦𝗘𝗔𝗦;
– Original & photocopy of identification card (refer to the list of valid ID cards as provided in the application form);
– 4 pieces original recent passport-sized ID picture (refer to the specifications written at the back of the application form);
– Original & photocopy of Certificate of Live Birth (only for applicants without date of birth in their I.D. card/s);
and
– 𝗣𝗮𝘆𝗺𝗲𝗻𝘁 𝗼𝗳 𝟱𝟬𝟬.𝟬𝟬 to CSC Field Office – Oriental Mindoro, Brgy. Sta. Rita, Pinamalayan, Oriental Mindoro 5208.
2. Please be advised to report to the CSC Field Office – Oriental Mindoro on your scheduled date of appointment.
